Welcome to Clearfield High School. The following information will help you complete your registration for the 2019-2020 school year. Your student WILL NOT BE REGISTERED until ALL 3 required steps/forms have been completed and returned to Clearfield High School. Please click on the tabs below to complete your registration. ALL REQUIRED FORMS are due to the main office August 1st, 2019.
Step 1. SIGNATURE PAGE: (Sign and return)
Print, sign and return to Clearfield High School
Step 2. REGISTRATION FEES
Complete one of the following options:
Option 1: Fees can be paid online through the my.DSD link. Sign in to your Guardian Account click "Payments", then click "Pay for Fees" then "Fees/Fines Owed" tab to pay online.
Option 2: Fill out a PAY PLAN AUTHORIZATION FORM and return to the CHS Main Office.
Option 3: FEE WAIVER APPLICATION (if applicable).
Fee Waiver Applications must have income verification attached. The Davis School District requires proof of eligibility. (See fee waiver application for approved categories).
Fee Waiver Applications are due September 24th, 2019. Students who register after the due date have one week from enrollment to submit an application.
Fee waivers are not retroactive.
** A $10.00 late registration fee will be added on August 15th, 2019
Step 3. STUDENT INFORMATION FORM: (Sign and return)
Log on to my.DSD, this will take you to the "Student Information System." Click on your student attending Clearfield High School. Then, click on the "Academics" link on the top of the page, then click reports. Print, update, and sign the "Student Information Form" and return to the school. This must be done even if there are no changes.
