2023-2024 Student Registration Instructions

Welcome to Clearfield High School. The following information is for ALL students except new students (incoming sophomores that attended Legacy, North Davis, Sunset and West Point are not considered new students) and will help you complete the registration process. This process confirms that you will be attending our school for the 2023-24 school year. Click on the steps below to see information. Please complete this registration process by August 4, 2023.

IMPORTANT INFORMATION

Your student is not registered and may not be able to see her/his schedule until this registration process is completed. Schedules will be available August 4th at 4:00 pm.
The spring course request process is different than this registration process.
Please pay registration fees by August 4th to avoid a $10 late fee.
All registration will be done online. *** Paper registration packets are not available*** (Except for new students who should click the 'New Student Registration' tab on the left.)
Computer access is available at the school for the registration process during office hours.
Back to School Night will be held Tuesday, August 15th from 5:30-6:30 p.m. - watch for the weekly newsletter email with further details.
Sophomore Orientation will be held Thursday, August 10th from 10:00 a.m. - 12:00 p.m..
First day of school is Thursday, August 17th.
To ensure that no student is denied the opportunity to participate in programs, courses, or activities because of the inability to pay you can find information on fee waivers and qualifications here.

SUMMIT LEARNING PROGRAM

This year our course offerings using the Summit Learning Platform are limited. Only Mr. Mike Handy’s 10^th Grade English classes, and Mr. Casey Oliverson’s 10^th grade World History classes will use Summit Learning.

If you want to be sure your sophomore student gets into these Summit Classes, please complete this form.

It is imperative that we have current guardian and student information accurate in our system. Log into your myDSD account to view information.

- Under the Tools tab, select Emergency Contacts and Pickup Authorization. If you need to add or take off any individuals, click here to fill out a change form.

GUARDIAN EMAIL AND PHONE NUMBERS
- To see the guardian contact information we have on file, click on your name at the top right corner. Select Account, if you need to change anything you can type in the new information and click save changes.

- If you have moved recently, click here to fill out an address change form.

You can verify that you have completed this registration step by signing the Review and Update Guardian Contact Information electronic document on myDSD.

Complete ONE of the following fee payment options by August 3, 2023
- OPTION 1
  - Pay fees online through myDSD. We will also accept payment in the CHS Main Office.

- OPTION 2
  - Request a payment plan contract (requires 25% down payment)
    - Complete a Payment Plan Contract and return it and the 25% down payment to the CHS Main Office.
- OPTION 3
  - Complete a FEE WAIVER APPLICATION (if applicable) and return it to the CHS Main Office

Review and accept Davis School District and Clearfield High School Policies through myDSD.

Log in to myDSD.
Click on the 'tools' tab on the top of the page and select 'sign documents' from the drop-down menu. (see graphic below).
Review each policy and click the green 'accept' button.

**This step must be done online. Computers will be available during regular CHS office hours.***

School pictures will be taken August 15-16, 2023. Please sign up for an appointment on your myDSD account. This picture will be used for your student's ID picture card. Students are required to wear their school issued ID at school visible from the front above the waist.

If you cannot come on those dates, there will be a makeup day on August 24, 2023. You will not need to make an appointment for this day because the students will be called down by last name during the school day.

There is no fee to get your picture taken. If you desire to Bell photo will be taking pictures, please see flyer for details below to purchase picture packages.

We are required by Federal Law to update the McKinney-Vento data base every year. Please complete this form regardless of your status. Your answers will help determine if the student meets eligibility requirements for services under the McKinney-Vento Act. Please fill out one form per family.

OPTIONAL REGISTRATION ITEMS FOR 2023-2024 SCHOOL YEAR

Please become familiar with these items. They are not required for your child to be registered.

Student Parking Permits

We have a limited number of student parking permits and conduct a lottery application to distribute the permits fairly. Please see the requirements to fill out an application below. Deadline to enter the lottery is Friday, August 4th. Student parking permits cost $5.00/semester. You will be invoiced for $5.00 for the 1st Semester and $5.00 for the 2nd Semester. Payment needs to be made before you may pick up your permit. Permits will be available to pick up in the office beginning Monday, August 14th.

Students with F's or outstanding U's from 22-23 second semester will not be eligible to purchase a parking permit.
Sophomores are not allowed to purchase a parking permit.
Students must have a parking permit to park in the school parking lot. There is parking available across the street from the school at Fisher Park if you do not have a permit.

Schedule Changes

Schedule Change Days:

Seniors: Monday, August 7.
All Students: Tuesday-Thursday August 8-10.

Class schedule changes are by APPOINTMENT ONLY. Schedule change appointments are made through myDSD and will open August 5. If there are no appointments available, we will work to open additional appointment times. Appointments will be held in the library so please park in the south parking lot by the cafeteria.

Appointments are brief (10 minutes), so come prepared with a completed Class Change Request Form. KNOW WHAT YOU WANT.

Schedule changes require a $10 fee. The student account will be invoiced and the fee can be paid online through myDSD or in person in the main office.

Schedule changes are subject to class availability, so a change may not be possible.
Students with the following scheduling issues will NOT be charged:
- Holes
- Duplicate classes
- Davis Tech Programs

If you feel like your request will take longer than 10 minutes or it is a simple change request, please email your assigned counselor.

Yearbooks can be purchased through myDSD or in the Main Office. The cost is $50. After October 31 the price goes to $55. Please don't wait to order your yearbook, we ran out of 2022-2023 copies.

Child Find Notice

Davis School District is responsible for Child Find identification and evaluation for all students suspected of having a disability residing in Davis County. This includes children (birth through 21 years of age) who are in public schools, private schools, are being home schooled, are preschoolers (birth to five years) and are in need of an evaluation for possible special education or early intervention services. Early identification and intervention may be essential to your child’s development.

If your child is having significant difficulty with vision, hearing, speech, behavior, is experiencing slow development typical for his/her age, physical development or learning difficulty, he/she may be a child with a disability that requires specialized instruction through special education. If you suspect that your school age child may have a disability, please contact your neighborhood school principal or special education department at 801-402-5169.

If your child’s age is birth to three years or three to five years, contact the Davis Early Learning Center at 801-402-5409.

Keys to Success Program

Clearfield High School is proudly participating in the Keys to Success Program. This incentive based program is designed to award students for their academic improvement. Your student will have the opportunity to win various prizes throughout their high school years for their academic achievements as well as the chance to win a grand prize and vehicle at the end of each academic year. Your high school’s Keys to Success Contact Person (Ms. Linford) will be implementing the program at your school. The Keys to Success program will run from September through May. The grand prize and final vehicle giveaway will occur between March and May.

Participation in the Ken Garff Keys to Success car giveaway is voluntary and is open to all students enrolled at the high school in 10th, 11th or 12th grade and who have submitted a signed Parental/Guardian Release Form to the high school.

PTSA Membership

Dues to join the PTSA can be paid in myDSD. Here are the membership options:

Parent membership fee: $7
Student membership fee: $7

Becoming a member of the PTSA does not require volunteer time, but if you are interested in getting involved there are lots of opportunities! The PTSA at CHS provides a meal for the teachers at each of the parent-teacher conference sessions, as well as sponsors teacher appreciation week in the spring. We also host the all-night graduation party, which provides a safe and fun environment to celebrate our new graduates. As the saying goes, many hands make light work, and with the help of wonderful volunteers we are able to make possible these opportunities and more! We would love to have your participation at whatever level you are comfortable with, whether it is bringing a one-time salad or helping at the graduation party.
